Or do only people with REALLY low self-esteem have all three of these feelings? Is it possible to have low self-esteem and not have all three symtpoms?

From my experience , yes they do. Really low self esteem is a condition where you experience self loathing, you can not escape it and you feel ashamed for just being who you are. Self loathing comes from childhood abuse, and being told you are worthless and horrible, from those who are supposed to love you. The only way to overcome this is to know what is ahead of you, understand why you feel the way you do.. and slowly understand that you deserve to be treated with respect, dignitity and are a beautiful person.
