If low self esteem is at the bottom, and confidence is at the top, what lies in between the two?

Optimism and Satisfaction.
If you have low self-esteem you are not happy with yourself or your life and therefore you will not be a happy person. It will reflect in every aspect of your life (spouse, job, money, goals etc). This is the biggest stumbling block to a fullfilling life.
If you have confidence that is great. But when you let confidence rule your life you can become cocky, arrogant, and superior. This will not only affect the way you view the world and other people it will affect how they view you. You may not have many friends and your job will suffer. These types of people are not well-liked and generally, end up depressed. You can not keep up the cloak of superiority very long. Someone better than you will always come along and knock you off your seat.
So the happy medium is Optimistic and Satisfied. Being in the middle yet looking at the bright side in every situation helps you advance, have friends, and keep ahead. Good energy creates good energy. Optimism creates optimism.
Always look on the bright side and your life will be brighter.
You are no better than anyother human living on this Earth but you can make better choices.

Humility. When you’re confident in who you are yet still know that you can always improve on yourself is when you’ve reach humility.

They are 2 different things and you can’t scale them. It’s like comparing apples to oranges. Self esteem (self confidence, self respect) is something that even shy people can have. Confidence is something some people sport to make people think they have high self esteem.
If you like/love yourself even just a little bit, you’re on the right track.
